I am Kisakye peace,19years old i sat for my PLE at St Andrews Nakabango primary school in 2013,UCE in 2017 at Lingira living hope s.s and UACE in 2019 at Goodheart s.s I would like to add on my passlips from P7-S6 a third name which is Kisakye peace Pollyana and to also change my date of birth from 26/06/2000 to 26/11/2000 and I have all my results from ple ,uce and uace .if there is away, I would like to know the procedure.

I am kisakye peace pollyana 19years old , I would like to know if it is possible to add a third name which is pollyana and change of date of birth on my passlips from 26/06/2000 to 26/11/2000 ,from P7 to S.6 .I sat my primary seven in 2013index 002277/037 at St Andrew's Nakabango,S4 in 2017 index U2778/021 at Lingira Living Hope s.s and my S.6 in 2019 at GoodHeart s.s .i would like to add a third name and change my date of birth so that i may have matching documents.
I will be very delighted if my request is put into consideration.
